The benefits of positive practice environments on patients, nurses, and organizations are well documented. They impact everything from nurse engagement and satisfaction to care delivery and patient outcomes.


It is also increasingly evident that wise investment in nursing services is the most effective way to achieve excellence. Studies show that positive practice environments ultimately save money – in the form of improved staff retention and decreased turnover, improved nurse and customer satisfaction, improved quality and safety, and superior business results.


The cost of replacing a nurse is high – anywhere from one to four times that of a nurse’s average salary. So LTC organizations that are able to improve nurse satisfaction and retain a high quality nursing staff realize a significant return on investment.


What is the Path to Excellence Program and the optimal nursing practice environment?


ANCC’s Pathway to Excellence® Program recognizes a healthcare organization’s commitment to creating a positive nursing practice environment. At the program’s core are 12 practice standards that impact the quality of LTC.


Based on evidence and expert nurse input, the standards represent qualities that both nurses and researchers agree are critical to high quality nursing practice, professional development, and job satisfaction. They include shared governance and collaborative multi-disciplinary decision-making at all levels, autonomy in practice, a safe and healthy work environment, professional development opportunities, fair compensation, work-life balance, and an organizational commitment to quality improvement and evidence-based practice.


Some features that make the Pathway to Excellence Program unique include: benefits to nurses (recognizing what is important to nursing practice, demonstrating to the community a commitment to nurses and their practice and inviting nurses to join their colleagues in a desirable and nurturing environment) and benefits to patients (improved unit effectiveness, quality patient care impact by the nurse’s ability to problem solve, collaborate and handle conflict, and patient safety outcomes – influenced by the quality of a nurse’s work environment, mediated with burnout and engagement).

What changes do you predict for the LTC sector?


Our growing aging population will fundamentally transform LTC delivery over the next 30 years. Up to a third of the population will soon be over 55, and the demand for LTC will increase dramatically. We must work now to create the positive practice environments and care delivery models that will prepare us to meet the needs of an aging population with chronic disease.
 
As we look forward, we see an emerging community of LTC organizations that extends around the world and a new generation of global “gold standards” that guide quality patient care. Having engaged and dedicated nurses is the key to making this vision a reality.

Most skilled nursing facilities have not yet switched to electronic medication administration record (eMAR) systems, missing out on tools that could help nurses achieve the five rights of medication safety and give facilities the ability to capture all charges, according to Tim Renz, Chief Executive Officer, & Mona Gamboa, Chief Technology Officer, CareRight Technologies.


From a solution provider at the marcus evans Long-Term Care CXO Summit Spring 2012, Renz and Gamboa discuss how to eliminate medication administration errors in long-term care (LTC) facilities.


How can LTC facilities eliminate errors and inaccuracies?


Tim Renz: Mistakes happen daily in LTC facilities that use paper-based MARs. Nurses are pulled in 20 different directions as they try to complete their daily responsibilities. A family member might ask them a question, a doctor may call or a resident could need their help. Then, they struggle to remember where they left off, and when they sit down to go through their charts to document everything at the end of the day, they forget to make a note of an important piece of information.


Mona Gamboa: These errors can be prevented. Technologies such as MedRight give nurses the tool to achieve the Five Rights of medication safety: right patient, drug, dose, route and time. It is simple for nurses to use on different devices, including portable devices. The proper administration of drugs is one of nursing’s primary responsibilities, but they are in the dark ages without the right tools.


With budget cuts in almost all sectors, why should LTC facilities budget for such a technology?


Tim Renz: Not all eMAR systems are expensive, and they have proven to increase efficiency, safety and save on nursing overtime. For example, nurses who utilize our technology do not need to spend one to two hours after their shift documenting their rounds. Medication administration is tracked automatically throughout their day.


MedRight automatically captures all the changes associated with each patient’s medication profile throughout the day, including treatments, procedures, and other orders. LTC facilities with paper MARs are not always able to keep track of medication changes efficiently.


How does this program help LTC facilities be patient centered?


Mona Gamboa: The trend is to be patient centered, to help people transition from one home to another. LTC facilities are no longer considered institutions and “patients” are now referred to as “residents” and “people”. The true patient centered facility will keep all of the resident’s belongings right in his room, including medication, in a locked cabinet accessible only by the nurse. Using the eMAR, residents can take their medications on their time and not the institution’s time. True “Point-of-Care” resident services with programs like MedRight® and CareRight® are easily achievable throughout the care areas of a facility.

All Electronic Medical Records (EMRs), will soon be connected and translated to mobile devices, says Jonathan Levoy, VP of Business Development & Technology, Alego Health. “Doctors must be able to get information when they are on the road visiting facilities, whether it be on a tablet device or smart phone,” he adds.


Ahead of the marcus evans National Healthcare CXO Summit Spring 2012, taking place in Hollywood, Florida, April 26-28, Levoy discusses the criticality of efficiency and mobile devices when implementing EMRs.


How can healthcare executives implement EMRs while making an impact on their bottom line?  


It is easier to train a clinician to do technical work, than teach an Information Technology (IT) major to do clinical work. The implementation of mobile devices can easily be accomplished through in-house employees; there is no need to spend time and money searching for high-end consultants. Utilize local talent that have experience and an interest in IT.


Instead of keeping the focus on the software and IT, it is advisable to make it about customer service and the experience of the physicians.


How can EMRs be used more efficiently?


Healthcare executives must first get used to the fact that paper records will soon no longer exist and EMRs are here to stay. They must work with the physician to ensure that they are comfortable with the software. The biggest challenge is getting used to doing things differently. The program must be user friendly so that users can process results and utilize the software as intended.


Efficiency is created by usage. Clinicians must initially get comfortable with the program. Giving too much to a person at once is detrimental to the overall success of the implementation. Healthcare executives can then go back and work on efficiency, number of clicks and learn how to navigate screens better.


Trying to teach a new product to someone is a difficult task. It is important to optimize the product, making it faster and easier to understand the program itself initially.


Are mobile devices currently being used to access EMR?


A lot will happen in the hospital environment in the next five years around efficiency and mobile devices.  


Soon, all EMRs will be connected and translated to mobile devices. Doctors must be able to get information when they need it. Speed and efficiency are paramount in the implementation of EMRs. Also the ability for physicians to access patient information from anywhere will make the transfer from paper to electronic storage more appealing to physicians as well as increase the level of patient care. These solutions are currently being offered through the use of tablet PCs and smart phones. As the use and experience level grows through these technologies so will the efficiency in day to day hospital operations.
